Terms and Conditions

Last Modified: October 2, 2026

1. Acceptance of These Terms

These Terms and Conditions ("Terms") are an agreement between you and Clover Meds LLC ("Clover Meds," "we," "us," or "our"). They govern your use of clovermeds.com, the Clover Meds patient portal, and any other websites, applications, or services we make available (together, the "Services").

By using the Services, you agree to these Terms and to our Privacy Policy, which is incorporated by reference. If you do not agree, do not use the Services.

You must be at least 18 years old and able to enter into a binding contract to use the Services.

2. Changes to These Terms

We may update these Terms from time to time. The "Last Modified" date above shows when they were last changed. If we make a material change, we will notify you by email or through the patient portal before it takes effect. Changes apply going forward and do not affect orders already placed or disputes already noticed. If you continue to use the Services after a change takes effect, you agree to the updated Terms. If you do not agree, you may stop using the Services and cancel your treatment plan as described in Section 13.

3. Eligibility

To use the Services, you must:

  • Be at least 18 years old, and meet any additional age requirements for a specific treatment;
  • Reside in, and be physically located in at the time of care, a state where the Services you request are available;
  • Provide accurate and complete information; and
  • Have a compatible device and internet access. Your carrier or internet provider may charge fees.

Meeting these requirements does not guarantee that you will receive treatment. Your provider decides whether a treatment is appropriate for you.

If you move or travel outside a state where your treatment is available, tell us before your next order. Providers can only treat you while you are located in a state where they are licensed and the treatment is offered. If you move permanently to a state we do not serve, we will cancel your plan and refund any prepaid months not yet sent to the pharmacy at your plan's monthly rate.

4. Our Services

Through the Services, you can complete a health questionnaire, communicate with a licensed healthcare provider ("Provider"), receive a prescription if your Provider determines it is clinically appropriate, and have your prescription filled by a state-licensed pharmacy.

Providers exercise their own independent professional judgment in all clinical decisions, including whether to prescribe, which medication to prescribe, dosing, monitoring, and whether to continue treatment. Clover Meds does not direct or interfere with any Provider's clinical judgment. No promotion, discount, referral program, or business consideration affects treatment decisions.

Clover Meds is not a pharmacy. We do not compound, manufacture, dispense, package, label, or ship medications. Prescriptions are filled and shipped by independent, state-licensed pharmacies. We place orders with the pharmacy on your behalf and coordinate fulfillment and payment.

Treatments available through the Services may include compounded semaglutide, compounded tirzepatide, NAD+ injections, and testosterone therapy. Compounded medications are not FDA-approved, and FDA does not review them for safety, effectiveness, or quality. Specific treatments may require separate informed consent, laboratory testing, follow-up visits, and additional eligibility criteria.

A provider-patient relationship begins only when a Provider reviews your information and accepts you for care. Browsing our website, using our virtual assistant, or submitting a questionnaire does not by itself create a provider-patient relationship.

Clover Chat and other messaging are not monitored 24 hours a day and must not be used for emergencies. Providers respond during business hours.

You authorize us to share your information with Providers, pharmacies, laboratories, and payment processors as needed to deliver the Services, as described in our Privacy Policy.

5. Your Account and Your Information

You must create an account to receive treatment. You sign in using a one-time code sent to you; keep access to your email and phone secure, and tell us promptly at [email protected] if you believe someone has accessed your account without permission.

You give us permission to use the information you provide, including health information ("Your Information"), to deliver and support the Services, as described in our Privacy Policy. We do not sell Your Information, and we do not use or disclose your health information for third-party research without your separate written authorization.

Our website uses cookies and similar technologies, including advertising and analytics tools, as described in our Privacy Policy. We configure these tools so that they do not send your questionnaire answers, treatment, prescriptions, or other health information to advertising platforms.

6. Code of Conduct

You agree to:

  • Provide accurate, complete, and current information, and update it if anything changes;
  • Use the Services only for yourself; and
  • Not misuse the Services, interfere with their security, infringe anyone's rights, or encourage others to violate these Terms.

We may suspend or close accounts that violate these Terms.

7. Medical Disclaimer and Medication Risks

The Services are limited to the specific treatments offered and do not replace a primary care provider or comprehensive medical care. Talk to your primary care provider about your overall health and any other conditions or medications.

All prescription medications, including compounded GLP-1 medications (such as semaglutide and tirzepatide), NAD+, and testosterone, can cause side effects, including serious ones. Side effects vary by person and medication. Information on our website is not a complete list of risks and does not replace advice from your Provider. Before starting treatment, review your treatment consent, any education materials we provide, and the pharmacy label and instructions. Results vary, and no specific outcome is guaranteed.

If you have severe symptoms or think you may be having a medical emergency, call 911 or go to the nearest emergency room immediately.

8. Telehealth Consent; Specialty Treatments and Controlled Substances

Telehealth uses electronic communication to connect you with a Provider in another location. It has benefits and limitations. Before receiving care, you must review and accept our Telehealth Informed Consent.

Some treatments, such as testosterone therapy, involve controlled substances. Prescriptions are issued only at your Provider's discretion, when clinically appropriate and permitted by law. You agree to complete any treatment-specific consent, laboratory testing, and follow-up your Provider requires. If you do not complete required monitoring, follow-ups, or labs, your Provider may change, pause, or stop your treatment.

9. Privacy

Your privacy matters to us. Our Privacy Policy explains how we collect, use, and protect Your Information.

10. Fees, Payment, and Insurance

Prices are shown on our website and at checkout. Your price includes your Provider's care, medication, supplies, and shipping. There are no other fees.

By completing checkout, you authorize us and our payment processors to charge your payment method for the amounts shown, including recurring charges for month-to-month plans as described in Section 12. If a payment fails, we will ask you to update your payment method, and your next order will not be placed until payment is received.

Billing questions and chargebacks. If you think a charge is wrong, please contact [email protected] first; we respond within two business days and correct any error. If you dispute a charge with your bank or card issuer, we may pause future orders while the dispute is open and may share the records needed to respond (such as your order, payment, and acceptance of these Terms) with the payment processor. We will not share more health information than necessary to respond.

We do not accept insurance and are not in network with any health plan, including Medicare and Medicaid. Medications and lab services ordered through the Services may not be covered by your insurance. We do not accept HSA, FSA, or CareCredit cards directly; on request, we can provide an order confirmation you may submit for reimbursement.

11. Our Pricing Commitments

  • Same price at any dose: your price does not change if your Provider adjusts your dose.
  • Price lock: your price for a treatment stays the same for as long as you continue that treatment, even if our advertised prices change. For month-to-month plans, this means as long as your plan stays active. For prepaid plans, you keep your price if you buy your next plan for the same treatment within 30 days after your current plan ends. If you cancel, or let more than 30 days pass after a prepaid plan ends, current prices apply when you restart.

12. Treatment Plans, Billing, and Shipping

Provider approval first. No medication is ordered until a Provider approves your treatment and you confirm your treatment plan. If you are not approved, you receive a full refund.

Month-to-month plans. Month-to-month plans renew automatically until you cancel. Each order is placed with the pharmacy about three weeks after your previous shipment, so you do not miss doses, and your payment method is charged when the order is placed. We send a reminder about five days before each order. You can view your next order date and adjust it by up to two weeks in the patient portal.

Prepaid plans (3, 6, or 12 months). Prepaid plans are charged in full at checkout and do not renew automatically. Medication is shipped in increments based on the treatment and pharmacy limits:

  • Compounded semaglutide and tirzepatide: shipments of up to 3 months of medication;
  • NAD+: shipments of up to 2 months of medication;
  • Testosterone: monthly shipments.

Every shipment requires your Provider's continued authorization. Compounded semaglutide and tirzepatide patients complete a Provider follow-up every three months. If a required follow-up or lab is not completed, your next order will not be placed or charged until it is.

Availability. Medication availability depends on pharmacies and on federal and state law, which can change. If a medication cannot be filled because of a shortage, pharmacy issue, or regulatory change, we will tell you promptly and refund any prepaid months not yet sent to the pharmacy at your plan's monthly rate. Your Provider may also discuss other clinically appropriate options with you.

Delivery. Once your Provider approves treatment, your order is sent to the pharmacy, and medication typically arrives within 7 business days. The carrier emails your tracking number when your order ships. Delivery times are estimates and depend on the pharmacy and carrier.

13. Cancellations and Refunds

How to cancel. You may cancel your treatment plan at any time, with no cancellation fee. The fastest way is in the patient portal: go to the Orders page and select "Cancel Treatment." Portal cancellations take effect immediately, and you will receive a confirmation. You may also cancel by emailing [email protected] or through Clover Chat; please write at least 3 days before your next order date. An emailed or chat cancellation takes effect when we receive it.

Orders already sent to the pharmacy. Once an order has been sent to the pharmacy, it cannot be cancelled, changed, or recalled, and it is non-refundable, except under our 30-Day Money-Back Guarantee (Section 14), if we made an error, or where the law requires a refund.

Month-to-month plans. Cancelling stops all future orders. If you were charged for an order that had not yet been sent to the pharmacy when your cancellation took effect, we refund that charge.

Prepaid plans: if you cancel. If you cancel a prepaid plan, we refund the amount you paid minus the month-to-month price for each month of medication already sent to the pharmacy. The refund will not be less than zero. Example: you prepay $273 for 3 months of semaglutide ($91 per month); one month has been sent to the pharmacy; the month-to-month price is $98; your refund is $273 - $98 = $175.

Prepaid plans: if your Provider stops treatment. If your Provider decides treatment is no longer appropriate, or you become ineligible for a medical reason, we refund the remaining months not yet sent to the pharmacy at your plan's monthly rate, with no deduction.

Not approved. If a Provider does not approve treatment, including after laboratory testing, you receive a full refund.

Medication unavailable. If a medication cannot be filled as described in Section 12, we refund prepaid months not yet sent to the pharmacy at your plan's monthly rate.

Refunds are issued to your original payment method, typically within 5 to 10 business days. Program fees are not a stored balance or cash equivalent and do not carry forward except as described in these Terms.

14. 30-Day Money-Back Guarantee

Who is covered. New patients, for the first month of any treatment we offer (compounded semaglutide, compounded tirzepatide, NAD+, and testosterone), on month-to-month or prepaid plans. The guarantee is available once per patient for each treatment.

How much is refunded. The full price you paid for your first month, with nothing deducted. For prepaid plans, the first month equals your plan total divided by the number of months in the plan, and any months not yet sent to the pharmacy are also refunded at your plan's monthly rate. Additional months that were already shipped (for example, when a 3-month supply ships at once) are not refunded under this guarantee.

How to request it. Email [email protected] or message us in Clover Chat within 30 days after your first shipment is delivered.

Returning remaining medication (semaglutide, tirzepatide, and NAD+). After your request, we will email you return instructions. Return all remaining medication, opened or unopened, within 7 days of your request, at your own cost, using a trackable shipping method. We issue your refund after we receive and inspect the return.

Testosterone (no mail-back). Testosterone is a controlled substance and cannot be mailed back. Instead, within 7 days of your request: (1) photograph your remaining medication, (2) dispose of it following FDA guidance on medication disposal (a drug take-back location is preferred), and (3) submit the photo and a signed disposal attestation, which we will send you. We issue your refund after we receive both.

Requirements. You must have completed your Provider consultation and confirmed your treatment plan. After the first month, charges are covered by the cancellation and refund rules in Section 13, not by this guarantee.

15. Laboratory Testing (Testosterone Therapy)

Testosterone therapy includes initial and ongoing laboratory testing, at no additional cost. Your Provider may order labs through a third-party laboratory before or during treatment. These tests are used to evaluate whether testosterone therapy is appropriate and to monitor your treatment. They are not a comprehensive health screening and should not be relied on to diagnose or rule out other conditions. Please follow up with your primary care provider about any concerns or abnormal results.

Completing labs or a consultation does not guarantee a prescription. If your Provider does not approve treatment, you receive a full refund.

If you complete laboratory testing but do not attend your Provider consultation and confirm your treatment plan within 30 days after your results are available, despite our reminders, your order may be cancelled without a refund, because the laboratory services have been performed. We may decline to order testing where there is evidence of misuse.

16. Order Delivery and Claims

If there is a problem with your delivered order (for example, damaged, missing, or incorrect items), report it to [email protected] within 24 hours of the recorded delivery time. The pharmacy may not accept claims reported later. We will submit a claim to the pharmacy on your behalf and help it get resolved.

Pharmacies are responsible for the preparation, handling, quality, and shipping of the medications they dispense.

17. Promotions, Referrals, and Price Match

Promotions. Promotional codes and discounts apply as described in the offer. Unless an offer says otherwise, additional discounts apply to the first month only, and promotions may be changed or ended at any time for future purchases.

Referral program. Each patient receives a referral code. A new patient who signs up with a referral code receives the discount stated in the current offer, and the referring patient receives the stated account credit after the new patient is approved by a Provider and completes a first purchase. Referral credits have no cash value, are not transferable, and may be applied only to eligible purchases. We may limit, change, or end the referral program, and may revoke credits obtained through fraud or abuse. Referral credits are never contingent on, and never influence, any clinical decision. Clover Meds does not participate in Medicare, Medicaid, or other government healthcare programs.

  • For the same active ingredient, formulation, dose, and quantity;
  • A month-to-month price (prepaid multi-month offers, first-month prices, and introductory rates do not qualify);
  • Publicly advertised and verifiable; and
  • From a licensed U.S. provider or pharmacy that has operated legally for at least one year, for human use.

Price Match Guarantee. If you find a lower price for an equivalent offer, we will match it and take 20% off your first month. To request a price match, sign up at our current price and send proof of the lower offer to [email protected] (or through Clover Chat if you are already a patient). If the offer qualifies, we issue a partial refund and adjust your price going forward. To qualify, the competing offer must be:

We review each request individually. Price match benefits are not transferable or redeemable for cash.

18. Accuracy of Your Information

Your Provider relies on the information you provide to make safe treatment decisions. You agree that all health and personal information you provide, including your medical history, current medications, and measurements such as weight, is truthful, complete, and current, and you will be asked to confirm this. Providing false or incomplete information can put your health at risk and may result in your treatment being denied or stopped.

19. Electronic Communications and Text Messages

Electronic notices. You agree to receive agreements, notices, receipts, and other communications from us electronically, by email, text message, or through the patient portal. These communications satisfy any legal requirement that they be in writing. Keep your email address and phone number current.

Text messages. By providing your mobile number, you agree to receive text messages from Clover Meds about your account and care, such as sign-in codes, order and shipping updates, refill and follow-up reminders, and messages from our team. If you separately opt in to marketing texts, we may also send offers and promotions; agreeing to marketing texts is not a condition of any purchase. Message frequency varies. Message and data rates may apply. Reply STOP to opt out of text messages at any time, or HELP for help; you can also contact [email protected]. If you opt out, we may still contact you by email or through the patient portal, and you may not receive sign-in codes or reminders by text. Text messages are not encrypted, so we keep health details in texts to a minimum. Carriers are not liable for delayed or undelivered messages.

20. AI Virtual Assistant

Our website uses an AI virtual assistant to answer general questions about our services. It does not provide medical advice. If anything the assistant says conflicts with these Terms or with instructions from your Provider, these Terms and your Provider's instructions control. For questions about your care, contact your Provider through the patient portal.

21. Intellectual Property and Trademarks

The Services and their content, including text, graphics, logos, images, and software, are owned by Clover Meds or its licensors and are protected by copyright and trademark laws. You may use the Services only for your personal, non-commercial use. "Clover Meds" and our logos are trademarks of Clover Meds LLC and may not be used without our written permission. Other trademarks that appear in the Services belong to their respective owners, who are not affiliated with Clover Meds unless stated. If you send us feedback or suggestions, we may use them without obligation to you.

22. Third-Party Links

The Services may link to websites we do not control. We provide links for convenience only and are not responsible for third-party websites or their content.

23. Disclaimer of Warranties

TO THE FULLEST EXTENT PERMITTED BY LAW, THE SERVICES, INCLUDING THE WEBSITE, PATIENT PORTAL, AND THEIR CONTENT, ARE PROVIDED "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICES WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE OF VIRUSES OR OTHER HARMFUL COMPONENTS. MEDICATIONS ARE PREPARED AND DISPENSED BY INDEPENDENT PHARMACIES, AND CLOVER MEDS DOES NOT MAKE ANY WARRANTY REGARDING THEM. SOME STATES DO NOT ALLOW THE EXCLUSION OF IMPLIED WARRANTIES, SO SOME OF THESE EXCLUSIONS MAY NOT APPLY TO YOU.

24. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY LAW, CLOVER MEDS AND ITS AFFILIATES, OFFICERS, EMPLOYEES, AGENTS, LICENSORS, AND SERVICE PROVIDERS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR FOR LOST PROFITS, REVENUE, DATA, OR GOODWILL, ARISING OUT OF OR RELATED TO YOUR USE OF THE SERVICES, WHETHER BASED ON CONTRACT, TORT (INCLUDING NEGLIGENCE), OR ANY OTHER THEORY. TO THE FULLEST EXTENT PERMITTED BY LAW, OUR TOTAL LIABILITY FOR ANY CLAIM RELATED TO THE SERVICES WILL NOT EXCEED THE AMOUNT YOU PAID US IN THE TWELVE MONTHS BEFORE THE CLAIM AROSE.

NOTHING IN THESE TERMS LIMITS OR EXCLUDES ANY LIABILITY THAT CANNOT BE LIMITED OR EXCLUDED UNDER APPLICABLE LAW.

25. Indemnification

You agree to indemnify and hold harmless Clover Meds and its affiliates, officers, employees, and agents from third-party claims, losses, and expenses (including reasonable attorneys' fees) arising from your breach of these Terms, your misuse of the Services, or your violation of any law.

26. Governing Law and Dispute Resolution

Governing law. These Terms are governed by the laws of the State of Wyoming, without regard to conflict-of-law rules, and by the Federal Arbitration Act.

Informal resolution first. Before starting arbitration, you or we must send a written Notice of Dispute describing the dispute and the relief requested, including (for you) your name, address, and the email or phone number on your account. Notices to Clover Meds go to: Clover Meds LLC, Attn: Dispute Notice, 1309 Coffeen Ave, Ste 1200, Sheridan, WY 82801. We will both try in good faith to resolve the dispute within 30 days.

Binding arbitration. If the dispute is not resolved within 30 days, it will be resolved by binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, in Palm Beach County, Florida, or by video or telephone if the arbitrator allows. Arbitration fees are governed by those rules. YOU AND CLOVER MEDS WAIVE THE RIGHT TO A JURY TRIAL.

Small claims. Either of us may instead bring an individual claim in small claims court if it qualifies.

Opt out. You may opt out of this arbitration agreement by mailing a written opt-out notice to the address above within 30 days after you first accept these Terms.

Court. Any claim that is not subject to arbitration, or if you opt out, will be brought only in the state courts located in Palm Beach County, Florida, or the U.S. District Court for the Southern District of Florida, and you and we consent to their jurisdiction.

This Section survives the end of your relationship with Clover Meds.

27. Class Action Waiver

All disputes will be resolved on an individual basis only. Neither you nor we may bring or participate in a class, collective, consolidated, or representative action or arbitration. If this class action waiver is found unenforceable for a claim, that claim will proceed in court, not in arbitration.

28. Time Limit to Bring Claims

Unless the law requires a longer period, any claim related to these Terms or the Services must be brought within one year after it arises, or it is permanently barred.

29. Termination

You may stop using the Services at any time and cancel your treatment plan as described in Section 13. We may suspend or end your access to the Services if you violate these Terms or if we can no longer provide the Services to you. If we end your access for reasons other than your violation of these Terms, we will refund any prepaid amounts for medication not yet sent to the pharmacy. After termination, we will continue to provide access to your health records and any continuing care as required by law and professional obligations. We retain records as required by law. Sections 7, 13, 14, 21 through 28, and 30 survive termination.

30. General Terms

Entire agreement. These Terms, our Privacy Policy, your Telehealth Informed Consent, and any treatment-specific consents are the entire agreement between you and Clover Meds about the Services. If they conflict, the treatment consent controls for clinical matters and these Terms control for everything else.

Severability. If any part of these Terms is found unenforceable, the rest remains in effect.

No waiver. Our failure to enforce any part of these Terms is not a waiver of our right to enforce it later.

Assignment. You may not transfer your rights under these Terms. We may transfer ours in connection with a merger, acquisition, or sale of assets, with notice to you.

Events beyond our control. We are not responsible for delays or failures caused by events outside our reasonable control, such as natural disasters, carrier or pharmacy disruptions, or internet outages. Any refund rights in Section 13 still apply.
